Senate Bill 12 Required Consent Forms
Due to new legislation for the 2025-26 school year, parents are now required to opt-in or opt-out of certain services that have previously been provided to students. Forms can be located in Parent Portal.
Health Services and Counseling Services forms need to be completed for each student PK-12. Forms for club consent only need to be completed for students in grades 6-12.
